Works Permit Application Form

Before beginning this form, please ensure that you have prepared your supporting documentation as this is required before you can submit your application. If you are unsure what supporting documentation you need, please call our works permits officer on 08 8930 0597 to have an initial discussion about the works you are proposing to do.

Application must be received 5 business days prior to requested start date. Road closures require a minimum notice as per the conditions of approval. Works permit is not valid without payment AND signature of authorised officer. 

No works are to proceed prior to receiving approved Works Permit.
